Help Transitioning From the Hospital to Home
We begin planning your recovery at home even before you leave the hospital or inpatient rehab. By collaborating with discharge planners, we help ensure you have a smooth return home and an easy transition to our home care program so that you will continue to get the care you need.
Short-Term Rehab

MJHS Menorah Center

Located in Manhattan Beach, Brooklyn
Set on 10 beautiful acres in Manhattan Beach, Brooklyn, MJHS Menorah Center is a 436-bed skilled nursing center providing exceptional inpatient rehabilitation and long-term care.
Memory Care
Our teams empower and reconnect memory-impaired residents with their passions through specialized non-pharmacological treatments, sensory activities, memory stimulation, and other therapeutic experiences. MJHS Menorah also offers a dedicated memory care residence for patients and families.
Specialized Hospice Care Wing
Located on the grounds of Menorah, the MJHS Mollie and Jack Zicklin Hospice wing offers residents receiving hospice care the option to stay in one of 16 spacious suites. This specialized wing provides a comforting environment for patients and a welcoming space for family and friends to spend meaningful time together.
MJHS Isabella Center

Located in Washington Heights, NY
Situated on four lush acres in Washington Heights, NY, MJHS Isabella Center is a 705-bed rehabilitation and nursing center specializing in care for seniors, individuals with disabilities, and their families. We provide comprehensive inpatient rehabilitation and long-term care tailored to diverse needs.
Ventilator-Dependent Care
MJHS Isabella Center offers certified ventilator-dependent beds and a specialized interdisciplinary team, delivering comprehensive respiratory care and rehabilitation services.
Isabella House
On the campus of Isabella, we offer independent living options for adults 62 and older. Each bright, spacious, newly remodeled studio or one-bedroom apartment is moderately priced and features spectacular skyline views, providing a comfortable and vibrant living experience.

Medicare Advantage Plans
Designed for Medicare-eligible individuals looking for comprehensive coverage beyond Original Medicare. Plans include benefits like hospital, medical and prescription drug coverage along with vision, dental, hearing, and more.
Special Needs Plans
Plans tailored for individuals with Medicare and Medicaid that provide dedicated care managers to coordinate your care. Benefits include hospital, medical and prescription drug coverage along with vision, dental, hearing, and more.
Managed Long-Term Care Plans
