Please note, that while we provide an extensive range of services, we do not currently accept patients requiring infusion therapies, maternal and child health services, behavioral health services, or occupational therapy in the Manhattan area.

Skilled Nursing Care
Once you are admitted into our home care program, our skilled nursing care team will work with you and your private physician to coordinate and develop a plan of care that includes specific goals and objectives.
Your skilled nursing care team can:
- Help manage your medications
- Educate you and your caregiver on managing disease-specific conditions such as heart failure, diabetes or chronic obstructive pulmonary disease
- Supervise personal care provided by a home health aide
- Administer shots and educate family members on tube feedings
- Change dressings
- Coordinate with our palliative care team to provide you with enhanced services if you are experiencing pain and suffering due to an advanced illness
Rehabilitation

The MJHS Home Care, Road to Recovery Rehabilitation program has proven expertise in helping patients recover from hip or knee replacement surgery, stroke, spinal cord injury or any other orthopedic or neurological condition that affects mobility.
Our licensed physical and occupational therapists and language pathologists provide the following:
- Comprehensive, evidence-based treatment plans
- Aggressive, individualized goal-setting to help patients reach their peak mobility and activity levels
- Home safety and accessibility evaluations with modification recommendations
- Advanced techniques to reteach patients essential skills such as bathing, dressing, walking, stair climbing, balance and cooking, and to use equipment such as walkers, canes, wheelchairs and other ADL devices
The MJHS Road to Recovery Rehabilitation program provides:
- Critical rehabilitation services provided by licensed physical and occupational therapists and speech-language pathologists
- Comprehensive, evidence-based treatment plans
- Aggressive, individualized goal-setting to help patients reach their peak mobility and activity levels
- Culturally sensitive services
- Home safety and accessibility evaluations with environmental modification recommendation

- Relearning essential skills such as bathing, dressing, walking, stair climbing, balance and cooking

We also offer subacute rehabilitation at Menorah Center for Rehabilitation and Nursing Care. Menorah is located on the ocean in Manhattan Beach, Brooklyn, and offers private rooms for most of our subacute patients.

Our programs aim to provide disease-specific education and care to patients and caregivers so they can develop the skills necessary to manage their diagnoses independently and prevent re-hospitalizations.
Heart Failure Disease Management
As the first home care agency in the country to receive Joint Commission certification for Heart Failure Disease Management, you can feel safe trusting us to help you manage your heart failure diagnosis safely and effectively at home.
Our clinicians are highly skilled in heart failure management. At the core of this program is working closely with you in the comfort of your home to better manage your disease and improve symptoms like shortness of breath. We utilize proven best practices, such as:
- providing more frequent skilled home visits earlier on in the care plan
- educating you and your family members about early recognition of symptoms
- reviewing changes in medication that may have occurred during a hospitalization
- ensuring that follow-up doctors’ appointments are made and kept
- providing ongoing support
- training you to promote self-management of diet, weight, stress and medication regimen

Chronic Obstructive Pulmonary Disease (COPD)
MJHS Home Care has a well-established disease management program for patients with COPD that is designed to improve symptoms and address the high level of hospital readmissions typically experienced by patients who have COPD.
This program utilizes proven best practices, such as:
- teaching patients how to control symptoms
- educating patients and their families about risk factors for disease progression
- advising patients about how they can reduce risks and maximize their own health outcomes
- educating patients on how to manage their medications
- providing techniques to avoid flare-ups and what actions to take should one occur
- providing available resources to help patients quit smoking
This program has achieved positive outcomes. Our patients have shown improvement in managing symptoms such as shortness of breath with the proper use of medications, resulting in reduced trips to the hospital.
Social Work
Our social work team plays a vital role in ensuring your physical, emotional and environmental needs are met. Our social workers are licensed in the state of New York and have master’s degrees in social work. Many are also bilingual and have had cultural sensitivity training. They are knowledgeable about all the programs we offer and can help patients transition from one level of care to another if needed.
They can also aid with:
- Counseling
- Additional services such as Medicaid applications and other entitlements
- Education regarding your treatment and the complexities and impact on your lifestyle
- Long-term planning
- Linking you to additional resources in the community
The frequency of social work visits is based on individual needs and will be determined in conjunction with the skilled nursing staff and your doctor.
Home Health Aides
To receive home health aide services, it must be part of the recommended plan of care prescribed by your doctor. Medicare only covers Home Health Aide (HHA) services if you are also receiving skilled nursing or rehab services.
If you have a Home Health Aide (HHA) as part of your care team, he/she is trained and certified to provide personal care that is essential to maintaining your health or to care for your illness or injury. Your nurse/therapist is responsible for establishing a Plan of Care for your Home Health Aide (HHA). The plan describes the care you will receive and the number of hours and days that service will be provided. They will be able to assist with activities of daily living (ADLs) such as:
- Grooming
- Bathing or showering
- Toileting
- Dressing
- Exercising as instructed by your nurse or therapists
- Feeding
- Walking
- Getting in our out of bed
- Changing positions in be
A Home Health Aide provides care for the patient–not the whole family.
Many of our aides are bilingual or multilingual, and we always do our best to match patients with a home health aide who speaks their language.

Does insurance cover home health care?
MJHS Home Care is paid in full or in part by Medicare, Medicaid, managed care providers (including HMOs), private insurance, other third-party payers or private pay.
Since coverage varies, check your plan’s Evidence of Coverage booklet, website or customer service center for more information. We are also happy to help you determine your health insurance status.
