Medication management is a fundamental part of the long-term care we provide. If you’re receiving care at home, your team can assist with medication reconciliations (to avoid dangerous drug interactions or overmedication),  scheduling your medications,  providing education about the purpose of each prescription and its potential side effects, and the importance of taking your medications as prescribed. 

For residents at our two skilled centers, on-site pharmacies reconcile and provide medications, which are administered by a nursing team member to ensure safety and optimize health outcomes.

What Is Medication Management?

Medication management involves a coordinated approach to ensuring individuals take their medications safely and as prescribed. This includes:

  • Medication Review and Reconciliation – Ensuring all prescribed medications are accurate, necessary, and safe to take together.
  • Scheduling and Reminders – Establishing medication schedules and, for individuals living at home, a process to remind patients or their caregivers when it’s time to take a medication.
  • Monitoring for Side Effects and Interactions – Identify potential adverse reactions, advise the treating physician and facilitate necessary adjustments.
  • Education and Support – Providing members and their caregivers with information about proper medication use, potential side effects and the importance of taking medications as prescribed.
