This list represents some of the common qualifying indicators for admission to our Certified Home Health Agency.
We can help you with your patient’s needs assessment.
- Anemia, B12 injections
- Care of indwelling urinary catheter
- COPD
- Diabetes
- Drainage tubes
- Exacerbation of chronic illness
- Heart disease
- High blood pressure
- Hip or knee surgery
- Hospital re-admissions
- Injections and infusions
- Kidney disease, dialysis
- Lower extremity fractures
- Multiple emergency room visits
- New medication and/or medication management
- Newly diagnosed chronic illness (Parkinson’s)
- Ostomy care
- Pain management
- Post-operative care
- Rehabilitation therapy
- Stroke (new or symptoms of late effects)
- Trach care, oxygen
- Unsteadiness, frequent falls
- Wound care
